DESCRIPTION:With federal youth reentry requirements for Medicaid/CHIP enrol
 lment taking effect now, justice leaders have new tools to ensure that you
 ng people leaving juvenile detention and jails are connected to care to im
 prove their return to the community. This seminar will highlight how count
 ies can move from complex policy requirements to functional, real-world op
 erations so that participants leave with concrete next steps and answers t
 o their implementation questions.\n\n\nThrough the Stepping Up Initiative,
  we have been working directly with county jail leaders to create clear, a
 ctionable approaches tailored to local needs, with a priority on engaging 
 rural facilities not yet involved in important implementation conversation
 s. We help jails strategize how to approach state Medicaid agencies, manag
 ed care organizations, and community partners so that they can stand up th
 eir programs, build IT and data-sharing infrastructure, and strengthen ope
 rational workflows grounded in best practices across the field.\n\n\nThe s
 ession will feature early implementation efforts from 1-2 county jail site
 s focused on operationalizing the requirements for young adults in custody
 . Presenters will share practical lessons on building capacity, aligning o
 perations with Medicaid and managed care, and ensuring sustainable connect
